Eligibility: M.Pharma, Pharm D
Experience: 3 to 6 years of relevant experience
Job Description:
- Rational use of drugs
- Consultation regarding the selection of drug, dose, route, dosage form, schedule, and duration.
- To provide Information regarding
- Drug action, drug kinetics,
- Drug interactions (drug-drug, food-drug),
- Contraindications and special precautions,
- Management of patients on polytherapy (multiple medications)
- To ensure and monitor drug safety
- Adverse drug reaction (ADR) monitoring
Hospital Drug Formulary
- To initiate, coordinate and accomplish the task of forming our own drug formulary and to see that it is implemented
- Audits
Medication Error
- High-risk drug compliance audit, narcotic, surgical prophylaxis,
- NABH MOM Standard implementation, audit, compliance and improvement
- JCI MMU Standard implementation, audit, compliance and improvement
- Suggestions to improve the quality of patient management (drug prescription, dispensing, administration, outcome) in various units/departments (according to hospital standards)
- Drug utilization studies, drug audits (mainly antimicrobials) in ICUs to know the pattern of antimicrobial prescription vs microbial growth and sensitivity. This will help in the control of antimicrobial resistance
